Bridge Event Reporting Form
This Bridge Event Reporting Form is designed to collect incident and near-miss information from vessel operators. Completed forms will be used in support of AWO's work to prevent bridge allisions and other incidents by investigating and documenting vessel operator's issues encountered when calling or transiting moveable bridges (drawbridge, swing bridge, lift bridge, etc.).
What can I report?
- Communication issues with bridge tender.
- Other issues that affect safe navigation.
- Bridge waiting too long to open when a vessel approaches.
- Bridge not opening all the way.
- Bridge not opening on time.
- Brdige closes too early.
- Bridge lights not operational.
